Care Navigator Trial

The National Aged and Community Care Roundtable for Forgotten Australians (National Roundtable) Care Navigator Subcommittee (Lotus Place, Tuart Place, Forde Foundation and Global Community Resourcing) collaborated with Council on the Ageing (COTA) to secure the first Care Navigator placement for Forgotten Australians from January to December 2022. The subcommittee is committed to the measurement of the social impact that the delivery of the Specialist Forgotten Australians Aged Care Navigator Project will have on this community. All Consortium partners have concisely and thoroughly considered the impact that the specifically dedicated Care Navigators will make for Forgotten Australians as they age.


The collaboration represents a unique specialist approach to the delivery of the Care Navigator Trial and will ensure that the lessons learnt in the year of delivering services to Forgotten Australians will be shared at a national level to inform the rollout of the new Care Finders Project launching in 2023. The Care Navigator Trial will ensure that Forgotten Australians are made aware of and are assisted with specialist services tailored to their specific needs.


The Consortium partners look forward to continuing to deliver high-quality dedicated Care Navigator Services and the expansion of these services in the new Care Finders Program from 2023.
